Enroll in Return Assured

Upon submission of this application, dated 04/19/2024, Return Assured℠, P.O. Box 8505, Mandeville, LA 70470, (hereafter known as “RA”) acknowledges that subject to the terms and conditions herein, you, “The Company” (defined by information provided in this application, including individuals), shall be an authorized enroller of the Life Membership benefits further described below.

Now, therefore, RA agrees to provide persons enrolled by the Company for Life Membership the benefits as described below. All services are provided by American Pre-Arrangement Services, Inc. (hereafter knows as “APASI”) and must be arranged by APASI. No Claims for reimbursement will be accepted.

APASI’s Life Membership provides the following benefits:

• In the event of death of a Member who is more than 75 miles from their legal residence or in another country which is not their country of residence at the time of death, APASI will render every assistance, including locating a local, licensed funeral home, mortuary or direct disposition facility, arranging for the transportation of the body from the site of death to the licensed funeral home, mortuary or direct disposition facility in order for such licensed funeral home or facility to prepare the body for transport, purchasing the minimally necessary casket or air tray for transport, arranging transportation of the remains to the Enroller’s location or a licensed funeral home or mortuary in the city/state of the legal residence of the Member, and procuring the consular services (for death overseas) and death certificates.
• Set procedures for the time of need so there will be no added stress due to any uncertainly about what to do.
• Access via the Internet to overview trends, recommendations or warning regarding senior travel.
• Emergency message transmission

*Legal residence is defined as the place of residence of 180 or more days in the twelve months preceding the date of death. It will require verification through voter registration, driver’s registration, and/or other means. A nursing home will be deemed the residence if the stay there has exceeded 180 days.

If Member enrollment of APASI occurs while 75 miles away from the legal residence, return of remains benefit become effective upon the Member returns to the legal residence for subsequent travel.

CANCELLATION POLICY: Member may cancel membership within 30 days for full refund. NO refunds past 30 days.

APASI cannot be held responsible for failure to provide services for delays caused by strikes or conditions beyond its control, including, but not limited to, flight conditions, or where rendering of service is prohibited by local law or edict.

Legal actions arising hereunder shall be barred unless written notice thereof is received by APASI within 1 year from the date of even giving rise to such legal action.

In return for the Company’s participation as a General Agent Enroller (hereafter known as GA), RA shall provide a commission to GA for each accepted Member’s application, to be disbursed by GA’s to writing counselors at GA’s discretion. The election of specific benefits shall be made by individual application. Attached to this Agreement, GA shall provide RA a list all locations that are associated with their GA and all writing agents that are under their GA. Each writing agent will receive an ID associated with their GA. This ID will need to be written on each application in order for commissions to be disbursed correctly. RA will provide a 1099 Income Tax Form to GA only.

This agreement is of one-year duration from the date stated above, and is automatically renewable annually in the absence of 90 day notice by either party of intent not to renew. Completed agreements can be mailed, emailed or faxed as indicated below.
